All volunteers must have a valid CORI form  on file. 

A Criminal Offender Record Information (CORI) form is required to be completed and submitted by all parents prior to acting as a field trip chaperone/volunteer.  The CORI is a record of all criminal court appearances in Massachusetts for a particular individual, including arrests, convictions, dismissals, and serious violations.  The CORI check is good for 3 years from the date it is run.  The CORI form can be completed at any school office, the Central Office, or downloaded below and hand delivered to any school office or to the Central Office. Identification such a drivers license, must be presented along with the form to office staff when the form is submitted.
